There are several situations:
* The project is being done by a "team" of e.g. TCS / Infosys / blah and they *have to* bill the client for X number of resources, so they just hire the guy for $25 / 35 / 45 per hr and charge the client $100+ / resource. So once the guy has enough experience, he gets hired full time and gets paid more.

* The actual hiring manager has to exhaust certain amout of budget so people get hired and then they do misc. work.

* Hiring companies don't get anyone to do the job so they have to hire someone maybe sub-standard just to fill the position

And people do get hired based on resumes.
What I don't really get is how do the actual employers put up with the quality they get.

You know the situation here in the US is even worse with <strike>recruiters hunting</strike> head hunters. Half of them are desis sitting in india with a VOIP phone and they have no clue of what positions / technologies they are hiring for or even "where" the position is. All they know is how to use the search box to match the keywords in the position requirements and a candidates resume.

And people do get hired based on resumes. I have seen phone interviews with not much being asked other than what the candidate's hobbies were and what was the latest project about and how they liked New York.
